Residential Services
With MENTOR Oregon’s residential group homes, family members and friends can rest assured that their loved ones with brain injuries or mental health challenges are receiving the best care possible. We provide individualized support and the opportunity for adults to grow and live more independently with our experienced team by their side.
Our staff gets to know each individual and develops an Individualized Service Plan (ISP). We offer hands-on support and guidance with cooking, cleaning, laundry, personal hygiene and other daily living activities. We also help adults spend time enjoying social activities and community outings, such as:
- Trips to movie theaters
- Trips to local parks
- Hiking
- Bowling
- Shopping
- Eating out at restaurants
In each of our homes, we provide around-the-clock support that includes medication administration, skill development and transportation to medical appointments and mental health treatment sessions. In addition to our experienced direct support professionals, a community-based psychiatrist makes monthly home visits and our staff consults with mental health professionals as needed. This ensures that the adults we support get the most out of each day while maintaining healthy lives.
Modern Homes and Amenities
Our homes are warm and welcoming places where residents can feel safe and comfortable. We can also tailor our homes to fit the unique physical needs of each individual who lives in the home. Some of the features we’ve added include:
- Elevators
- Wheelchair ramps
- Lift belts
- Individual bathrooms
In addition to personal growth at home, we want individuals to be comfortable in the community. That’s why our homes are nestled in areas that provide easy access to public transportation, shopping, libraries and other community buildings. Some adults—depending on their ISPs—can also take advantage of independent time in the community, without being accompanied by staff.
Supported Living Apartments
MENTOR Oregon also supports individuals with brain injury or mental health challenges who are able to live independently or with a roommate in a private apartment. Adults who live in supported living apartments, provided by MENTOR Oregon, have demonstrated a capability to manage every day chores and personal obligations.
Our caring staff members encourage individuals to do as much for themselves as possible, with the peace of mind that support is there whenever the need arises. We provide hourly support based on each person’s needs, and ongoing encouragement to help individuals be successful in the following areas:
- Shopping
- Cooking
- Cleaning
- Money management
- Medication management
- Education
- Employment
- Health and wellness
- Scheduling
- Transportation
In addition to the hourly services we also ensure the overall health and safety of the individuals we serve through periodic room and safety checks as well as mediation between roommates when needed.
