Residential Homes
At MENTOR Oregon, we believe that a home is more than a house; it’s where we grow and develop lasting relationships with the people we call family. In our residential homes, we help adults with intellectual and developmental disabilities live as independently as possible in the communities of their choice. We also offer specialized supports for individuals with cerebral palsy and autism spectrum disorders, as well as those with medically complex challenges.

Personalized Service and Support
In our residential homes, also known as our Developmental Disability Residential program, we provide a variety of personalized support and service options. Our program is designed around the needs and interests of the individuals in each home—ranging from 24/7 in-home support to hourly assistance with planned activities.
Because of our flexible, person-centered approach, we provide all individuals in our residential homes with a level of care designed to help them thrive and feel at home. Our support services may include assistance with:
- Personal hygiene
- Cooking and housekeeping
- Accessing community
resources and events
- Coordination of transportation
- Nursing consultations
- Financial management
We support adults of all abilities—including those who are medically fragile—and work with community-based nurses who visit our homes to provide specialized services on a regular basis. Our community-based homes are wheelchair-accessible and feature shower beds and positioning tables for those who are unable to move without assistance.
We also offer apartment settings empowering individuals to become more independent while receiving intensive behavioral supports and other personalized assistance. Adults living in our apartments are able to share a unit with one other resident or live in a unit of their own depending on their needs. Each person has access to around-the-clock support from our professional staff.
Individualized Service Planning
When someone is ready to move into a residential home, we take the time to understand what they enjoy and what makes them unique. Our homes are staffed with Direct Support Professionals and other human services staff who develop Individualized Service Plans (ISPs) to help each person reach new milestones. We make sure the individuals move into homes that are suited for their unique needs and do our best to match them based on age, gender, and similar levels of care.
In all of our community-based homes which are carefully monitored by our quality assurance professionals, we provide 24/7 staff support, expert clinical services, and opportunities for individual growth.
